Is היה a form of the Divine name?
No.
היה is the verb "to be" (i.e. "is"). The name יהוה is related to it by the (folk?) etymology of Exod 3:14, although it looks more like it should be related to הוה (which may itself be connected to היה, depending on who you ask).
